[ERP-CLINICAS] feat: Setup backend build configuration
- Add package.json with Express, TypeORM, PostgreSQL dependencies
- Add tsconfig.json with TypeScript compiler options
- Update .gitignore with comprehensive exclusions
- Build validates successfully (28 TypeScript files)
