- Moved backend/ to apps/backend/ (submodule)
- Moved database/ to apps/database/ (submodule)
- Moved frontend/ to apps/frontend-web/ (submodule)
- Updated .gitmodules references
- Created apps/_MAP.md
- Updated CLAUDE.md v0.1.0 -> v2.0.0

490530393d fix: Correct .gitmodules paths and URLs 
- Fix paths from apps/* to root level (backend/, database/, frontend/)
- Update URLs to use -v2 suffix matching actual remotes
